1. Define the wearer, product, and fit framework
Start with the intended wearer and use case. Record the target market, adult or youth range, gender or unisex direction where relevant, body profile, underlayers, movement needs, garment category, and intended silhouette. A close base layer, relaxed hoodie, protective shell, team jersey, and insulated pant need different relationships between body dimensions and finished-garment measurements.
Name the fit reference that anchors the chart: an approved pattern block, buyer fit standard, authorized reference garment, previous approved style, body-measurement study, or a clearly described development direction. Record its size, material, finish or wash condition, date, and approval status. State which features it controls and which are being changed so the new chart does not silently combine incompatible references.

2. Select the base size and complete size range
Choose the base size used for pattern development and first fit review. It should represent a useful center or decision point for the intended range, but the correct choice depends on the product and buyer standard. Identify every sellable size and avoid treating adult, youth, tall, short, plus, or other blocks as one uninterrupted sequence unless the buyer's fit system intentionally does so.
Create a size map that shows the label on the garment, ordering code, pattern size, wearer or body reference, and any market-facing conversion. Confirm whether alpha sizes, numeric sizes, age labels, or regional conversions are identifiers or actual fit equivalents. If two labels use the same pattern, or one label needs a separate block, state that explicitly before grading and label artwork are released.

3. Define every point of measure without ambiguity
Assign each point of measure a stable POM code and a specific name. Write where the measurement starts, where it ends, whether it follows a straight line or curve, whether seams or edges are included, how closures and adjustments are positioned, and whether the garment is measured flat, relaxed, extended, fastened, or under another stated condition. Add a labeled diagram using the same POM codes as the table.
Break compound requirements into separate rows. Front and back body length, relaxed and extended elastic, pocket opening and pocket position, inseam and outseam, hood height and width, or bicep and cuff are not interchangeable. Where one measurement depends on another feature, name the construction reference or anchor point. Test the method by asking two reviewers to measure one sample independently and resolve any row that produces inconsistent interpretations.

4. Build the base-size measurement set
Enter finished-garment targets for the base size using one declared unit. Connect width, length, balance, opening, placement, and functional measurements rather than adjusting one dimension in isolation. For tops, chest width interacts with shoulder, armhole, sleeve, bicep, neck, and length. For bottoms, waist, hip, rise, thigh, knee, hem, inseam, and outseam must form one wearable pattern relationship.
Distinguish finished-garment measurements from body measurements, pattern measurements, and component dimensions. Add the intended ease or relationship to the wearer where it helps explain fit, but do not copy body dimensions into the garment column. Mark derived values and dependencies, and record whether seam allowance, turnback, elastic, rib, lining, insulation, binding, or another construction element affects the measured result.

5. Write grade rules across the size range
For each POM, state how the value changes from one size to the next. A grade may be constant across part of the range, change at a block break, remain fixed for a design feature, or use different increments above and below the base size. Show actual measurement values for all sellable sizes as well as the underlying rule so reviewers can detect an incorrect formula or unexpected proportion.
Review the grade as shape and function, not a spreadsheet exercise. Shoulder, chest, armhole, sleeve, hood, pocket, rise, thigh, knee, and hem may need different progression. Check whether decoration placement, pocket access, protective coverage, articulation, rib, elastic, hardware, and label position still work at the smallest and largest sizes. Plan sample or pattern review at the sizes that carry the greatest fit or construction risk.

Stretch materials, padded assemblies, elastic, curved seams, and washed garments can behave differently from stable flat components.
State whether tolerance is plus/minus, one-sided, or linked to another measurement, and whether it applies to the individual sample, size average, or production check defined by the quality plan. Avoid using tolerance to authorize a change in intended fit. A sample inside numerical tolerance can still reveal an unacceptable balance or movement issue, while a useful fit decision may require a documented chart revision rather than an informal exception.

Identify the sample with style, revision, size, date, and relevant material lot or reference so results are not separated from the item measured.
Use a report that places target, tolerance, actual result, variance, status, comment, reviewer, and date on the same row. Record the method or condition issue when a result cannot be compared fairly. Photograph fit and detail concerns with the POM or action code, but keep photographs as supporting evidence rather than a substitute for updated values, pattern comments, and written approval.

8. Review, approve, and release one controlled chart
Review measurements together with fit, movement, construction, material behavior, decoration, and product interfaces. Convert each comment into a numbered action that identifies the affected POM, pattern area, size, reason, proposed value or rule, owner, and required evidence. When one change affects related measurements, update the complete relationship rather than changing a single table cell without reviewing pattern balance.
Release one measurement-chart revision with the matching pattern or pattern comments, measurement diagram, graded values, material and finish references, sample identity, fit record, accepted exceptions, and approval status. Archive superseded versions and state which revision controls the next sample or bulk stage. If a material, wash, construction, pattern, size range, or fit direction changes, identify which values, grade rules, tolerances, and samples require renewed approval.
